Before Your Visit
Quick answers to help you prepare.
01Where are you located?
We are at 1450 Gateway Drive, Madera, CA 93637. Parking is in the building lot, and we are a few minutes from Highway 99.
02What are your office hours?
Monday through Friday: 8 a.m. to 6 p.m. Saturday: 9 a.m. to 1 p.m. Sunday: closed. For anything urgent outside those hours, call 911 or go to an emergency room.
03What should I bring to my first visit?
Bring a photo ID, your insurance card if you use insurance, and any records or imaging you already have. Wear clothes you can move in, such as what you wear to work.
04Can I talk to someone before I book?
Yes. Call the front desk. They can answer questions about visits, fees and insurance, but they cannot give medical advice by phone.
